La Grange D102 has partnered with Miles of Smiles, Ltd., to provide FREE preventive dental services at Forest Road. These services will be available to children in all grade levels. Services may include exams, cleaning, fluoride varnish, and sealants if needed. Exams will take place during school hours on Thursday, December 1st.
Consent Form: To receive services, a consent form needs to be filled out and returned to the Barnsdale Main Office or uploaded online to the registration form. The consent form can be found HERE. A form needs to be filled out for each child.
Forms must be submitted online or at the Barnsdale main office by Monday, November 14.
